Submit a BLSO Course Request

Complete and return the form to sponsor a course.



Preparing for Your Course

Order your materials at least two months prior to the day of the program. You will receive the course materials no later than two weeks from the date you submit your BLSO Product Order Form to the AAFP Member Resource Center.

Order early enough that you can distribute a BLSO syllabus to each participant at least one month prior to your course date. This will allow your participants ample time to read the syllabus and complete the required online pre-course exam. Instruct registrants to bring the syllabus with them to the course. 

Sample Timeline for Planning an BLSO Course

Multiple steps are involved in setting-up a successful BLSO Provider Course. Read the sample timeline below to walk you through all of the steps and discover how long it will take you to complete the process.

Timeline: 4+ months beforePlanning Schedule:

Complete and submit the BLSO Course Request Form (including faculty list).

 
Timeline: 4+ months beforePlanning Schedule: Receive course approval email, which will include the online pre-course exam link, BLSO Product Order Form and Memorandum of Agreement (MOA) as attachment.
Timeline: 4+ months beforePlanning Schedule: Sign the MOA and return it to the AAFP BLSO staff.
Timeline: 4+ months beforePlanning Schedule:

Complete and submit an online application for AAFP CME credit.

 

Please note: your application for CME credit and signed MOA MUST be submitted before you submit a BLSO Product Order Form.

Timeline: 3+ months beforePlanning Schedule: Confirm that all faculty members have a current BLSO instructor manual and syllabus. If they do not, you must include these materials on your BLSO Product Order Form.
Timeline: 3+ months beforePlanning Schedule:

Complete and submit the BLSO Product Order form to the AAFP Member Resource Center.

 

Please note: you are required to purchase a Course Material CD-ROM for each BLSO Provider Course.

Timeline: 3+ months beforePlanning Schedule: Obtain meeting space and hotel rooms, if needed.
Timeline: 3+ months beforePlanning Schedule: Obtain conflict of interest information and confirm faculty.
Timeline: 3+ months beforePlanning Schedule: Distribute BLSO-approved promotional brochures and accept participant registration.
Timeline: 2+ months beforePlanning Schedule: Send a BLSO syllabus to each registrant.
Timeline: 2+ months beforePlanning Schedule:

Email the pre-course exam link to all registrants (use the text provided in your course approval email).

 

Please note: registrants are required to score at least 80% and provide the exam summary page to the sponsor before attending the BLSO Provider Course.

Timeline: 1+ month beforePlanning Schedule: Distribute a BLSO instructor manual to each instructor.
Timeline: 2 days afterPlanning Schedule: Ship all rented mannequins back to the AAFP.
Timeline: 2-4 weeks afterPlanning Schedule: Complete and submit the BLSO Participant and Faculty Form to the AAFP. Instructor candidate will submit Instructor Candidate Evaluations, if applicable. 
Timeline: Upon receipt of all post-course materialsPlanning Schedule: After the AAFP's BLSO staff receives the required post-course forms, sponsors will receive course completion certificates for all participants and wallet cards for those who passed the course. AAFP staff will add CME credit for the BLSO Provider Course to participants' and faculty members' records.
