The following guidelines apply to all proposal applications.


Proposal applications must be submitted electronically by Thursday, November 3, 2016. Notification of an application’s acceptance or non-selection will be sent via email in mid-December 2016. There is no limit on submissions. If you submit more than five applications, you will be asked to identify your top five choices.

Presentation Title

Applicants may submit a presentation title on the online application form. This title will be added to the topic listed on the needs assessment (e.g., “Acute Coronary Syndromes (topic): My Aching Heart (presentation title)”. Presentation titles must be no more than 5 to 6 words in length and no more than 50 characters (including spaces).

Faculty Reimbursement Policies

Applicants who are selected to present will receive:

  • $550 per day stipend to defray hotel, food, and ground transportation costs; paid only for the day(s) on which the faculty member presents
  • $200 per topic for content development
  • $200 honorarium per 60-minute session
  • $200 team collaborations
  • Coach-class airfare from and to the faculty member’s home destination
  • Waiver of FMX registration fee

All compensation and reimbursement will be paid to the faculty member in the form of a check issued by the AAFP once a completed faculty reimbursement voucher is submitted after FMX.

Co-presenters (if any) are not eligible for reimbursement, compensation, or waived FMX registration fees. The honorarium will be paid only to the faculty member who submits the application. The AAFP does not divide the honorarium among the submitting faculty member and his or her co-presenters.

Conflict of Interest Form and Interest and Expertise Portfolio

The applicant must read the CME Policy and Procedures for Full Disclosure and Identification and Resolution of Conflicts of Interest and submit the CME Conflict of Interest Disclosure form (Option 1). The applicant must completely and accurately disclose all financial relationships he/she has had in the 12 months prior to the form completion date and will have 12 months into the foreseeable future with a commercial interest. Should the status change during the course of the application process or the activity, the conflict of interest (COI) disclosure form should be updated as appropriate COI  forms must be completed or updated in order for submissions to be reviewed.

Having a financial relationship does not preclude an individual from presenting. If any financial relationship is determined to be a potential conflict of interest as it relates to the topic(s) submitted for the CME activity, the applicant will be notified, and a resolution will be discussed. Conflicts of interest must be resolved prior to confirmation of the individual’s participation. Selected applicants who are invited to present a session at the AAFP FMX will not be confirmed until their COI disclosure forms are reviewed and any conflicts are resolved.

All relevant financial relationships (as determined by the AAFP) must be disclosed to learners, and AAFP staff will include that information in the PowerPoint presentation to be verbally noted and by the presenter at the beginning of the session, even if no conflict of interest exists or a conflict of interest has been resolved. Any individual who refuses to disclose relevant financial relationships will be disqualified from presenting.

The AAFP complies with the Accreditation Council for Continuing Medical Education’s (ACCME’s) Standards for Commercial Support, the American Medical Association’s (AMA’s) Council on Ethical and Judicial Affairs (CEJA) ethics policies, and other laws, regulations, and codes that pertain to conflicts of interest. All faculty members are required to read and comply with the AAFP’s CME COI policy. Noncompliance will result in honorarium penalties and one-year probation from a faculty appointment.

The Family Medicine Experience is a CME activity with AAFP and AMA credit. All sessions must comply with AAFP, AMA and ACCME rules and; therefore, must be educational in nature. If you have a financial interest, such as a book or a mobile app, it is permissible to mention only if it is educational in nature and done so in a fair and balanced manner.  Promotional messages of any kind are prohibited in CME. 

Applicants are strongly encouraged to complete the National Faculty Education Initiative (NFEI) online training( The goal of the NFEI program is to ensure that faculty members understand the difference between certified CME activities and commercial marketing/promotional programs. NFEI training is mandatory for all individuals selected to present at an AAFP-produced CME activity and must be completed prior to serving as a faculty member.

Each applicant must complete or update the AAFP's Interest and Expertise Portfolio (IEP). The Interest and Expertise Portfolio is a database built to match the right opportunities with the right people.

The online application form cannot be saved and completed later. If you cannot complete the application in one sitting, you may download and save the PDF version of the application(2 page PDF) and complete it at your leisure.

Needs Assessments

A needs assessment with learning objectives is available for each topic. Applicants are strongly encouraged to review the needs assessment for their topic(s) of interest before they complete a proposal application. Educational formats are indicated on each needs assessment. Applications for topics that are not listed on the 2017 AAFP’s FMX Proposal topic list will not be accepted.

Faculty Contact Information

After you enter your AAFP ID number, your personal contact information will automatically be pre-populated in the application form. Nonmembers who do not have an ID number should contact the AAFP Member Resource Center at (800) 274-2237 to establish one.

Required Information

Please be prepared to provide the following information on the application form  (view sample(2 page PDF)):

Section 1: Course Information/Design

  • Recommended Practice Changes: Briefly describe three recommended practice changes that you will challenge learners to implement as you summarize the key takeaways in the Recommended Practice Slide of your presentation.
  • Problem-Based Learning: Briefly describe your plan to teach a case-based/workshop-oriented session in a highly engaging, small group learning environment.  Also, describe your prior experience with teaching in this format.
  • Addressing Barriers: Briefly describe how you plan to address practice barriers to optimal patient management related to the topic(s) of this session.

Section 2: Practice Gap Validation

  • In addition to addressing the gaps identified in the needs assessment, what additional (if any) educational need(s) is this presentation intended to answer?
  • Based on your review of the needs assessment, briefly describe why any (if applicable) of the proposed gaps are less significant than the ones you proposed.
  • Based on your review of the needs assessment, as well as your own research and experience, which specific knowledge and/or practice gaps do you plan to focus on?
  • List at least one, but no more than five references.  Include: author, name of source, volume/issue, page number, or URL.

Section 3: Presentation History and Experience

  • List your current faculty appointment(s) or your current position.
  • List your specialty, if other than family medicine.
  • List up to five previous presentations made to a national audience and/or that are relevant to this topic. Include dates, type of audience, size, and evaluation score (converted to a 0-5 Likert scale where 0 is low and 5 is high). Failure to list scores may have a negative impact on your application.

All sections of the application must be completed prior to submission. Any sections left blank will indicate an incomplete submission and will not be reviewed.
