To the Editor:

I was impressed with the article about “SOOOAAP” charting [“Documentation Tips for Reducing Malpractice Risk,” March 2000].

Medicine has certainly evolved since the SOAP model was introduced in the 1960s. The medical records system should evolve as well, and more attention needs to be paid to this topic by U.S. medical schools. I plan to incorporate SOOOAAP into my practice immediately!
