Documentation Guidelines for E/M Services
These articles from FPM will help you to comply with Medicare's E/M documentation guidelines. They are based on the 1997 version of the guidelines. An earlier version of the guidelines was published in 1995. Medicare contractors base their audits on whichever version of the guidelines "is most advantageous to the physician," according to the Centers for Medicare & Medicaid Services. Note that FPM articles published within the past 12 months are accessible to AAFP members and paid subscribers only.
