Commonwealth Fund’s Melinda Abrams Joins TransforMED Board of Managers

LEAWOOD, Kan. – TransforMED LLC today announced Melinda Abrams, assistant vice president at The Commonwealth Fund, was appointed to serve a 3-year term on its board of managers. TransforMED is a wholly-owned subsidiary of the American Academy of Family Physicians, focused on helping primary care medical practices adopt the patient-centered medical home model of care.
Abrams serves as director of The Commonwealth Fund’s Patient-Centered Primary Care Initiative, which was launched in 2005 to spur the redesign of primary care practices and the overall health care system around the needs of the patient. Abrams is a leading expert on primary care -- specifically a new model in which all Americans would have a medical home that provides better access and effective care coordination within the context of an ongoing doctor-patient relationship.
Throughout her career, Abrams has also taken part in efforts to reform state policies regarding preventive health care to address early childhood development. She also has served as senior program officer for the Child Development and Preventive Care Program. She coordinated the Fund’s Task Force on Academic Health Centers, the Commission on Women’s Health, and The Commonwealth Fund/Harvard University Fellowship in Minority Health Policy. Abrams also has sat on the advisory committees of various national organizations, including the Child and Adolescent Health Measurement Initiative and the National Initiative for Children’s Health Quality.
