Become a Tobacco Aware Practice: Using an Organizational and Team-based Approach
Participants will learn to:
- Develop practice systems that work smarter--not harder--to increase interventions.
- Define staff roles and responsibilities for an effective team approach.
- Create a system to easily document tobacco cessation activities in EHR and paper records.
- Access evidence-based resources for educating staff, patients and community.
- Access and refer to community, state and national cessation support resources.
- Receive reimbursement for providing cessation treatment.
- Implement a smoke-free policy for your practice and promote such policies in your community.
Faculty:
Tim McAfee, MD, is the Chief Medical Officer for Free & Clear, a tobacco treatment provider for employers, health plans and government agencies, and was the family physician leader of a 10-year successful effort in an 800-physician integrated practice (Group Health Permanente) to decrease tobacco use in the half million population they served.
It is the policy of the AAFP that all CME planning committee/faculty/authors/editors/staff disclose relationships with commercial entities upon nomination/invitation of participation. Disclosure documents are reviewed for potential conflict of interests and, if identified, they are resolved prior to confirmation of participation. Only these participants who have no conflict of interest or who agree to an identified resolution process prior to their participation were involved in this CME activity.
