Tobacco Cessation Office Champions Resources

The AAFP has recruited 50 family medicine practices for our Tobacco Cessation Office Champions Pilot Project. Each practice has designated an Office Champion who, with training, developed a series of system changes to integrate tobacco cessation activities into daily office routines.

If you're a participant in the Tobacco Cessation Office Champions Pilot Project and you need to order additional quantities of any of the resources, please e-mail dswaffor@aafp.org.

Quitline Referral Cards – Refer patients to the National Network of Tobacco Cessation Quitlines: (800) QUIT-NOW, which will route patients to your state quitline for counseling and resources. Order plastic cards or print your own from pdfs.
