Evaluation of AAFP METRIC (Measuring, Evaluating, and Translating Research Into Care) with AAFP PEF (Practice Enhancement Forum) Program

Does your practice want to change any of its practice-care processes or interventions related to the delivery of care with one of these conditions:
  • Asthma
  • Coronary Artery Disease (CAD)
  • Diabetes
If so, consider joining this exciting evaluation project and help the AAFP determine if the AAFP Practice Enhancement Forum (PEF) or AAFP METRIC:
  • changed practice systems or processes related to better patient care outcomes
  • made improvements in patient processes and intermediate patient outcomes related to one or more of the above conditions due to participation in PEF and/or METRIC

Americans in Motion - Healthy Intervention (AIM-HI)

The AIM-HI project will study the effects of brief counseling and novel physiological measures on patients' physical and emotional health. Funded by a grant from the PepsiCo Foundation, AIM-HI will enroll 24 practices into a randomized controlled trial of the interventions. The Patient Sleep Problems & Alcohol Consumption Study

This cross-sectional “card” study will enroll approximately 100 clinicians and 2500 patients. Each clinician will collect data for 3 half-days. Eligible patients will complete an anonymous exit questionnaire about various sleep disorders and self-reported alcohol problems. Clinicians also complete a brief exit survey about the patient’s sleep problems, alcohol problems, and possible chronic diseases. The project objectives are to investigate: 1) the relationship between various manifestations of sleep disorders and types and severity of alcohol problems (alcohol dependence, risky drinking) of patients presenting to their primary care clinicians; 2) the clinician’s awareness of these two types of patient problems; and 3) the relationship between these two problems and selected chronic diseases (e.g., coronary artery disease, chronic obstructive pulmonary disease, hypertension, depression). Translating Research into Postpartum Depression Care

This project, a subcontract with Olmsted Medical Center and funded by AHRQ, will examine screening tools and the effectiveness of these tools when screening for post-partum depression in the family physician's office.
