Physician, Staff, and Patient-Reported Errors in Primary Care - DCERPS Pilot Study #1
Study Description and Methods
Five family physician offices and five family practice residency clinics were selected from the AAFP National Research Network membership that represented rural, urban, and suburban sites, as well as private and community health practices.
Participants from each office were asked to do a Sentinel Error Reporting for ten weeks and Intensive Error Reporting for five days. Each practice chose one of following schemes to report every single incident that they observe (Intensive Error Reporting):
-one of the ten weeks of the study period
-one day every other week for 10 weeks
The following reporting tools were used:
Web-based reporting tool (to be used by physicians, staff, and patients)
Telephone reporting tool (to be used by patients only)
Paper reporting tool (to be used by physicians, staff, and patients)

Specific Aims and Objectives
- To understand if family doctors, office staff, and patients will report medical errors and lapses in patient safety during a defined, intensive study period
- To compare patterns of errors reported by doctors, office staff, and patients
- To estimate errors in primary care practices as reported by physicians, staff, and patients
- To test and refine error-reporting tools
- To explore linkage and sequence of events, when possible
