Course Completion Requirements

Please email the completed required ALSO course forms below to the AAFP Program Coordinator for your course, within 2 weeks following course completion:
Once all course forms have been completed and materials returned to the ALSO staff, certificates will be emailed to the course coordinator to print and distribute to course participants. Provider Wallet Cards will be mailed to the course coordinator.

Please send, via regular mail, the Course Evaluation Forms -- Word file or PDF file, (7-page files; About Downloading, About PDFs) to:

Evaluation forms will be tabulated and an evaluation summary will be emailed to the course coordinator 6-8 weeks following the ALSO department’s receipt of the course evaluation forms.
Course sponsors should retain the written exam answer sheets and megadelivery score sheets for their files. Participant scores (number of correct answers, not rate of percentage) must be recorded on the Participant Information Form (2-page Excel file; About Downloading). Course participants and faculty will be entered and tracked in the ALSO database.

Per the AAFP payment guidelines, payment for invoices are due within 30 days of receipt. Course completion certificates will be issued after payment is received in full.
