2006 Certificate of Participation

Thank you for attending the 2006 Scientific Assembly. If you are an AAFP member, your credits have been added to your CME record. You can check your CME record and print a copy of it in the My Academy (Members Only) section of the web site. (Note: You will need your member ID number and password to log in.)

If you would like to print a certificate of participation for the CME you completed at Assembly, please choose from the two options below. After you print the certificate, write in your name and the number of credits you completed, then add your signature.
