Reporting CME After On-site Reporting Program Closes

Report your CME by phone
- Call Member Central at 800-274-8043.
- Have the session number ready for each course you attended at Assembly.
- Know whether you want to report full or partial credit for each session you attended.

Report your CME online
- Log-in to My Academy (Members Only). You'll need your AAFP ID and password.
- Select "Report My CME" from the options in the center.
- Choose "Locate the approved GROUP CME Course."
- Search for courses in October 2007, in Illinois.
- Find the courses for which you would like to report credit, and click "Add."
- When you have reported all your sessions, click the "Finish" button.

Print a Certificate of Participation

Attendees can also print a certificate of participation for the 2008 Assembly. Just select the correct participant type (physician or non-physician), download the PDF, add your name and the number of credits, and print the certificate.
