The Conference on Practice Improvement: Assembling the Patient-Centered Team is designed to help any practice setting begin or continue the work to create a medical home in their practice. The conference provides a forum for the reflective redesign of practice roles and processes through the team approach. Attendees are encouraged to bring interdisciplinary teams from their practices.
Some key conference topics will include group visits, advanced access, patient education and self-assessment, EHR readiness and selection, quality improvement, quality recognition, the idealized micro practice, and team care.
