AAFP Chapter Lecture Series
Learning Objectives
- Define the risk factors that predispose patients for type 2 diabetes and evaluate the current criteria for a diagnosis of prediabetes.
- Refine medical management skills so that treatment choices in patients with type 2 diabetes are appropriate for the stage of the disease.
- Evaluate how team-based care and the patient centered medical home (PCMH) model can be beneficial in the management of the type 2 diabetes patient.
