AAFP Scientific Assembly Faculty Forms

Please download all forms, review them carefully, and return to the appropriate staff contact by the designated deadline.

A faculty contract and conflict of interest form must be signed and on file prior to filling out the following documents. Please go to the Conflict of Interest Form and choose Option 1. AAFP ID number is required.

Each faculty member is required to submit an Intellectual Property (IP) Request for each presentation. Faculty should identify and list on the IP request form any third-party items used in the presentation. Information should include description of content, source, data/volume number, page number, author(s), publisher, and a copy of the original item from which the item was used. Faculty must disclose to AAFP and the learners when an unlabeled use of a commercial product, or an investigational use not yet approved for any purpose, is discussed during an educational activity. Faculty must disclose that the product is not labeled for the use under discussion or that the product is still investigational.
