CME Credit Statement

This activity has been reviewed and is acceptable for up to 1 Prescribed credit by the American Academy of Family Physicians. AAFP accreditation becomes 10-1-01. Term of approval is for one year from this date with option for yearly renewal. This activity has been renewed from 9-1-07 to 8-31-08.
