TransforMED received awards for best practices in three categories of establishing workplace learning and performance: culture, program design and development and technology innovation. Judging was based on the following criteria:
- Demonstrates achievement of organizational goals
- Provides evidence of programs that expanded the depth and breadth of employee knowledge and understanding
- Provides evidence of initiatives that provide continuous performance improvement
- Provides evidence of time savings
- Provides evidence of financial savings
- Provides evidence of high level in job satisfaction or retention
- Provides evidence of other measurable results in workplace learning and performance terms

Best Practices — Culture: TransforMED Model for Patient-Centered Medical Home
Since its inception in 2005, TransforMED’s expert practice enhancement facilitators have worked with primary care practices nationwide teaching them how to effectively manage and sustain change. The TransforMED model works on the premise that change planning involves everyone in the practice. Leadership, team work and communication practices are assessed early on as a means to determine change readiness. Transformation to the PCMH model changes the culture of the practice to a high performing one in which all staff work at their highest level of licensure resulting in improved professional satisfaction and financial health for the practice, as well as reduced costs, improved health outcomes and higher satisfaction among patients. As TransforMED continues to learn from its work with practices, additional transformation tools and resources are continually being developed for use by practice leaders.
Best Practices — Program Design and Development: TransforMED Patient-Centered Medical Home Transformation
TransforMED’s intervention design was based on learnings gleaned during the two-year National Demonstration Project in which 36 diverse family medicine practices attempted to adopt the PCMH model. Eighteen of the practices had full support from TransforMED’s practice enhancement facilitators, while the other 18 practices were given access to TransforMED resources but no expert facilitation. Many tools and resources were developed as a result of NDP findings, including a step one intervention “gap analysis” to determine where a practice currently stands on the journey to medical home transformation. Other tools developed include a change readiness survey, a patient satisfaction survey and a practice operations assessment. Program design and development are individually tailored to practices based on the baseline data collected. Adult Learning Principles are foundational to the development of tools and resources for transformation. In addition, periodic learning collaboratives are held throughout the year to bring practices together to learn from one another.
Best Practices — Technology Innovation: the TransforMED Delta-Exchange Social Network
TransforMED’s Delta-Exchange is a first-of-its-kind collaborative networking site dedicated to helping primary care practices implement the PCMH model of care. Key findings from the NDP revealed that primary care practices want and need opportunities to communicate, collaborate, and learn from other practices as they seek to overcome the challenges of implementing new technologies and workflows that are key to PCMH transformation. Delta-Exchange addresses these needs in an online, social networking platform. Delta-Exchange offers a host of individual member benefits. For a small monthly fee, members can create their own profile, post documents, share images and videos, create wiki pages and engage in discussions with other members. Members also gain access to optional, private work zones for physicians, clinical staff, office staff and others.
The PCMH model, through practice redesign and the increased use of health information technology, has been shown to reduce costs, improve access to care, increase patient satisfaction, improve health outcomes, and increase job satisfaction for primary care doctors and their staff.
