Family Medicine's Foundation

Family medicine’s core values and ideals provide a solid foundation for the patient-centered medical home. In our training and in our practices, family physicians consider the whole patient within the context of his or her family and community, taking into account the physical, psychological and spiritual components of health and disease. Our values reflect a comprehensive approach to patient service and embody the continuous healing relationship.

Some components of your patient-centered medical home will undoubtedly benefit from ongoing measurement and improvement, but if you’re practicing family medicine, you’ve got a head start. Download the PCMH Checklist (2-page PDF) to see whether other features of your patient-centered medical home need renovation; we’ve spotted you the basics:
  • Continuous healing relationships,
  • Whole person orientation.
  • Family and community context, and
  • Comprehensive care.
