Pay-for-Performance

Our Stance

The AAFP supports pay-for-performance (P4P) programs that focus on improved patient outcomes and provide positive physician incentives. The AAFP policy emphasizes seven principles for P4P programs:
  1. Focus on improved quality of care.
  2. Support the physician/patient relationship.
  3. Utilize evidence-based clinical guidelines.
  4. Involve practicing physicians in program design.
  5. Use reliable, accurate and scientifically valid data.
  6. Provide positive physician incentives.
  7. Offer voluntary physician participation.

AAFP Advocacy Activities

  • AAFP has met face-to-face with Aetna, Cigna, Humana, UnitedHealthcare and Wellpoint to discuss pay-for-performance and other issues. AAFP urges payers to adhere to the seven principles defined in the pay-for-performance policy.
  • In July 2006, AAFP sent a letter to Regence Blue Shield of Washington seeking a better understanding of its Regence Select Network, a pay-for-performance program that excluded family physicians. Read the letter (3-page PDF file; About PDFs).
  • In March 2005, the AAFP threw its support behind the National Committee for Quality Assurance's Bridges to Excellence program as a program that engages the patient while providing positive incentives for physicians. Read a press release about Bridges to Excellence and the AAFP's support.
