The DMAP fee schedule is currently updated monthly. Fees may change without notice. Please refer to the General Rules and other appropriate administrative rules and guidelines for payment policies.(Note: Fees are listed without decimal points: an item which is $22.64 will be shown as 2264.)
Information contained in the fee schedule is only to provide general information on DMAP's maximum allowable payment. For example, a procedure code listed with a fee does not mean that in all cases or in a specific case the service is covered. Providers must use all applicable administrative rules (OARs) to determine if there are any coverage criteria, limitations, restrictions, exclusion or client benefit limitations related to a specific procedure code. Also, while DMAP makes reasonable efforts to keep these schedules up-to-date, the actual payment amount, coverage and eligibility criteria in effect on the date of service will be applied, regardless of the general fee schedule information posted on this web site.
