Fee Schedule:

Utah Medicaid HCPCS and CPT Schedule - HCPCS and CPT codes with their Utah Medicaid payment amounts.  This is an Excel file compressed into a "zipped" file.

This file contains fees in the Medicaid data warehouse as of 10/13/2008 (the date of this posting) and reflects reductions mandated by the Utah Legislature for October 1, 2008.

Note: Beginning with the 9/16/2005 version of the fee schedule, the files are created as "zip" files.  Windows XP has a built in ability to extract the zip files.  Older versions of Windows will require an unzip program which can be downloaded free from one of the following locations.

     (4) Info-ZIP
     (5) The Unzip Wizard
     (6) The Ultimate Zip
Historical fee schedules (prior to 9/16/2005) are self extracting files (i.e. "*.exe files") and do not require an unzip program to extract.
