Comprehensive Primary Care Initiative
Led by the Centers for Medicare and Medicaid Innovation, the Comprehensive Primary Care initiative (CPC initiative) offers a blended payment model from public and private purchasers to primary care practices that provide "comprehensive primary care." As an incentive, the cost savings from this reform will be shared with participating physicians.
The areas selected to participate in the four-year demonstration project include statewide markets and partial state markets (by county).

Partial state markets by county include:
- New York: Capital District/Hudson Valley Region
- Ohio and Kentucky: Cincinnati/Dayton Region (New counties just added in Kentucky)

The AAFP and TransforMED are ready to assist family medicine practices in the selected markets that are interested in this initiative.
