AAFP has created these documents to assist members for the development of forms for requesting hospital privileges. The documents were developed based upon the core curriculum for Family Medicine and reflect the training offered in accredited family medicine residency programs. They are based upon Program Requirements for Graduate Medical Education (ACGME).
These files can be downloaded and modified by family physicians to develop standard privileges.
