Requesting a Corrected Evaluation
Please review your evaluation carefully upon receipt. Occasionally, an incorrect entry of submitted results may occur. If the error was made during the data processing and is a computer error that is not the fault of the participant, your entry will be re-evaluated if AAFP-PT is notified of the problem within one month of the evaluation date. The correction deadline date will be on the front page of the evaluation for each event. Corrections cannot be made after this date.
Sometimes the error is made by the participant completing the Result Booklet. Examples of this include situations where the booklet was filled out in ink instead of pencil, the computer scanning bubbles were not filled out in the booklet, the results were entered on the wrong page of the booklet, or the wrong instrument or method was entered.
Depending on the actual type of error, the result may be corrected and the Corrected Evaluation Request Form (1-page PDF file; About PDFs) should be sent to AAFP-PT. Specific instructions are found in the Result Booklet and it is the responsibility of the participant to complete the booklet correctly using these directions. Clerical errors made by the laboratory cannot be corrected. According to the CLIA 88 regulations, clerical errors are considered incorrect when performing proficiency testing. Allowable corrected evaluations are mailed approximately four to six weeks after AAFP-PT receives your request.
