Requesting a Replacement Evaluation

The CLIA 88 regulations state that it is the responsibility of the laboratory to keep copies of all PT documentation for two years. This includes copies of results submitted, the attestation signature and statement page, and all evaluations for each PT event (see Recordkeeping Requirements). Call AAFP-PT at (800) 274-7911 if replacement evaluations or copies are needed. Replacement evaluations and Result Booklet copies are mailed within two to four weeks; there is a nominal charge.
