Ambulatory Care Quality Alliance (AQA) Pilot -- This pilot, which is being funded by CMS and the Agency for Healthcare Research and Quality, will mark the first time that a combination of public and private information is being used to test approaches to measuring and reporting data publicly.

TransforMED -- Established in 2005 by the AAFP, TransforMED provides ongoing consultation and support to physicians looking to transform their practices to a model of care that is based on the concept of a relationship-centered personal medical home, as described in the Future of Family Medicine Report.
