Bridges to Excellence -- The Bridges to Excellence coalition is a not-for-profit organization created to encourage significant leaps in the quality of care by recognizing and rewarding health care providers who demonstrate that they deliver safe, effective, efficient and patient-centered care.

CMS Physician Quality Reporting System (PQRS) -- To participate in the PQRS, individual eligible professionals (EPs) may choose to report information on individual PQRS quality measures or measures groups.

TransforMED -- A subsidiary of the AAFP, TransforMED is available to assist practices looking to transform to a new model of care that is based on the concept of the patient-centered medical home.
