The ACF is an educational initiative aimed at bringing Academy members state-of-the-art clinical information in a specific area. ACF 2007 focuses on various aspects of managing chronic conditions, including the development of systems to assess care delivery and to improve chronic care quality, and incorporates the new model of care covered in the Future of Family Medicine Project report.
