Patients with diabetes often need disease-management services that extend beyond what can be reasonably provided during periodic office visits. Issues such as diet management and cardiac care are often difficult to summarize in a single visit, or even in two or three visits.

Now you can give your patients something to take home to help them optimize their self-management skills, thanks to the National Diabetes Education Program’s evidence-based online resources.

The NDEP Web site offers more than a dozen brochures, tip sheets and even a recipe guide for patients. Most resources are free for the first 25 copies and $5 for an additional 25. They may be freely duplicated and distributed to patients. The site also provides resources for health care professionals, school personnel and employers.

NDEP is a partnership of NIH, CDC and more than 200 public and private organizations. The AAFP is a member of the NDEP steering committee. The Academy's representative on the committee chairs its Health Care Providers Work Group.
