Upcoming Conference to Focus on Chronic Disease Management

This year's AAFP Conference on Practice Improvement: Health Information and Patient Education, scheduled for Nov. 9-12 in Denver, is all about helping family physicians better manage their medical practices and patients.

A joint presentation of the AAFP and the Society of Teachers of Family Medicine, the 2006 conference will feature a clinical focus on chronic illness management. Plenary sessions to be presented at the conference reflect that clinical focus and include
  • "From Barriers To Bridges: Improving Chronic Disease and Preventive Care" by FP Richard Wender, M.D., of Thomas Jefferson University in Philadelphia, who is president-elect of the American Cancer Society Board of Directors;
  • "Creating an Ideal Practice" by L. Gordon Moore, M.D., of the University of Rochester, N.Y., a solo practice family physician who also is involved with the Institute for Healthcare Improvement and the Reweaving the Safety Net project in Rochester; and
  • "How Can We Help Practices Redesign to Improve Care?" by Perry Dickinson, M.D., Marjie Harbrect, M.D., and Carolyn Shepherd, M.D., all of the University of Colorado in Boulder, Colo.
Two hands-on, skills-building preconference workshops will be held. They are
  • "How To Make Money For Your Practice: Billing for Patient Education" by AAFP Speaker Thomas Weida, M.D., of Pennsylvania State University in University Park, Pa., and Priti Mody-Bailey, M.D., M.A., of Community Medical Associates in San Antonio, Texas; and
  • "Evidence-based Educational Prescriptions for Patients" by Daniel Friedland, M.D., of the University of California, San Francisco.
Attendees at the conference, which was formerly known as the Conference on Patient Education, will learn how to
  • increase practical skills using health information and patient education to improve practice in all settings,
  • enhance interdisciplinary interaction and team developments that support practice improvement, and
  • encourage and assist patients in achieving optimal self-management.
