TransforMED Rolls Out New Product to Support Solo, Small Practices

TransforMED, a wholly owned subsidiary of the AAFP, has launched a new service that offers small primary care practices the help they need to implement the patient-center medical home, or PCMH, model of care.

According to a May 25 news release, TransforMED's Small Practice Package program "bundles together the necessary tools and components and streamlines the process to enable practices with four or fewer physicians to implement the components of the TransforMED PCMH model in two years."

The package is available to practices for $1,250 per quarter for virtual online support or $2,500 per quarter for practices that choose TransforMED's on-site option. Practices must commit to a two-year program enrollment.

Both the virtual and on-site options include unlimited access to Delta-Exchange, TransforMED's online learning community, and enrollment in the TransforMED Institute, a soon-to-be launched educational forum.

TransforMED President and CEO Terry McGeeney, M.D., M.B.A., told AAFP News Now that the release of the new package of services fulfilled TransforMED's long-standing goal of helping small family medicine practices become true patient-centered medical homes.

"The majority of AAFP members work in solo or small practices with two, three or four docs," said McGeeney. "In our work during the past several years, TransforMED has seen these small practices struggle. We're delighted to be able to offer these hard-working physicians practice tools that will improve their clinical outcomes, achieve cost savings and, for many, put the joy back into practicing medicine."

McGeeney pointed out that no two practices, regardless of size, are alike. "Each has a unique set of challenges," he said, adding that "TransforMED recognized early on that large practices, particularly those in thriving metropolitan areas, have better opportunities for peer-to-peer learning and networking, while smaller practices are oftentimes isolated." The new product offering should help level that playing field, he said.
