Free Delta-Exchange Webinar Features Family Physician Medical Home Champion

Family physicians considering a move to the patient-centered medical home (PCMH) model of care, as well as FPs looking for some help fine-tuning the processes they've already started in their PCMH practices, can get an assist from a family physician who has invested a lot of time in making the model work in Texas.

Amy Mullins, M.D., has been knee-deep in medical home matters since 2006 when her practice, Trinity Clinic in Whitehouse, Texas, was selected to join TransforMED's National Demonstration Project. Since the completion of that project in 2008, the PCMH model at Trinity Clinic has continued to grow and thrive.

Mullins will be speaking during a free one-hour webinar, Making the Medical Home Work, on Aug. 21 at 1 p.m. CDT. The webinar is being offered via the TransforMED Delta-Exchange network. AAFP members who are not signed up for their free Delta-Exchange network membership can do so before the webinar.

Mullins, who is the physician champion of the PCMH team at the Trinity Mother Frances Health System, will offer tips about operating a PCMH practice, including how to
  • keep team members engaged and working at the top of their game;
  • maximize health information technology to keep practices running smoothly; and
  • incorporate new ideas, such as electronic visits, into patient schedules.
