Transitioning Pediatric Patients to Adult Primary Care

Pickler, an assistant professor of family medicine, pediatrics and otolaryngology at the University of Colorado School of Medicine has established herself as a leader on the topic of care transitions. She helped write portions of a report titled "Supporting the Health Care Transition From Adolescent to Adulthood in the Medical Home" (21-page PDF; About PDFs) that was published in the July 2011 Pediatrics.
That guideline, a collaborative effort of the AAFP, the American Academy of Pediatrics and the America College of Physicians, provides detailed guidance on how to plan and execute optimal health care transitions for young patients.
In the one-hour webinar, Pickler will, among other things,
- review transition of care principles that are applicable to all children and adolescents,
- highlight portions of the 2011 report, and
- share pertinent points from a program called Got Transition? offered by the National Health Care Transition Center.
