Family physicians who want to learn more about risk-stratified care management can tune into a free webinar on that topic offered by TransforMED's Delta-Exchange network on Nov. 28 at 1 p.m. CST.
Webinar moderator Jeffrey Brenner, M.D., of Philadelphia, a family physician, is the director of the Institute for Urban Health at Cooper University Hospital. He's also the founder and executive director of the Camden Coalition of Health Care Providers, a nonprofit organization committed since 2003 to improving the quality, capacity and accessibility of the health care delivery system in Camden, N.J.
Brenner brings to the webinar a background steeped in helping ensure that people living in underserved communities receive high quality and personalized health care. He will help physicians understand how to
- identify high-risk patients in their practices,
- implement office systems that are able to monitor patients' risk status,
- access preventive care resources on behalf of patients and
- manage patients deemed to be at high risk.
