Integrate Risk-Stratified Care Management Into Your Practice

In the not-so-distant future, CMS and private health insurers across the country likely will expect family medicine practices to have systems in place to help identify which patients have serious health care conditions that require more monitoring, special services and an overall care plan.

Family physicians who want to learn more about risk-stratified care management can tune into a free webinar on that topic offered by TransforMED's Delta-Exchange network on Nov. 28 at 1 p.m. CST.

Webinar moderator Jeffrey Brenner, M.D., of Philadelphia, a family physician, is the director of the Institute for Urban Health at Cooper University Hospital. He's also the founder and executive director of the Camden Coalition of Health Care Providers, a nonprofit organization committed since 2003 to improving the quality, capacity and accessibility of the health care delivery system in Camden, N.J.

Brenner brings to the webinar a background steeped in helping ensure that people living in underserved communities receive high quality and personalized health care. He will help physicians understand how to
  • identify high-risk patients in their practices,
  • implement office systems that are able to monitor patients' risk status,
  • access preventive care resources on behalf of patients and
  • manage patients deemed to be at high risk.
With past experience as a solo FP in an urban practice, Brenner knows firsthand the challenges of providing full-spectrum health care services to poor and underserved patients. But, regardless of practice size or location, risk-stratified care management should be a topic of interest to all primary care physicians.
