ICD-10

CMS has released the final HIPAA-mandated code set implementation date for the International Classification of Diseases, 10th revision, Clinical Modification (ICD-10-CM) as October 1, 2015.

Note: CMS announced in July 2015 that it will grant a one-year grace period for the ICD-10 transition. This means that while the implementation date is still October 1, Medicare claims will not be denied based on which diagnosis code was selected as long as the physician submits an ICD-10 code from an appropriate family of codes. Medicare Care Coordination

Learn about coding and implementation on Medicare's cognitive care codes that allow you to improve your quality while also maximizing your payments.

Chronic Care Management
Through CPT code 99490, family physicians and other eligible health care professionals can be reimbursed by CMS for providing chronic care management (CCM) services to their Medicare patients who have two or more chronic conditions that are expected to last at least 12 months.

Transitional Care Management
Two CPT codes are used to report TCM services and are designed to pay for services provided to Medicare beneficiaries whose medical and/or psychosocial problems require moderate- or high-complexity medical decision making during transitions in care from a hospital or other health care facility to a community setting (e.g., home, rest home, assisted living).

Advance Care Planning
ACP is the face-to-face time a physician or other qualified health care professional spends with a patient, family member, or surrogate to explain and discuss advance directives. Two CPT codes are used to report ACP services: 99497 and 99498.
