With a holistic view of patient medical and social history, family physicians are uniquely equipped and professionally prepared to participate in pre- and post-operative assessment and care. A partnership between the family physician and the primary surgeon can help reduce the risks associated with surgery and the risk of postoperative complications, while considering all of the patient’s other conditions. Care coordination and transitional care services provided by the family medicine practice team play a pivotal role in ensuring continuity of care for patients, enhancing health care access, improving quality, and controlling costs.
