    Family physicians are doctors for all people of all ages and health conditions, providing reliable first contact and longitudinal health care. Family physicians help patients understand, prevent and manage illness; set health goals; and navigate the health system. Family physicians and their teams use principles of equity and justice to advocate for the unique needs of their patients and communities. Family physicians and their teams leverage data to manage patient populations effectively and employ evidence-based practices to optimize health outcomes while recognizing structural determinants of health to provide high-value, cost-effective care.
