This is true even when you correct for age, specialty, academic rank, leadership positions, publications, work hours, spousal employment and research time (Ash et al, 2004; Jena et al, 2016, Jagsi et al, 2012). Women are underrepresented in higher academic rank and published articles in prestigious medical journals (Jagsi et al, 2006). These recent studies have debunked the theories that the gap was based on a male gender bias for the higher paid specialities by evaluating a homogenous population of physicians (Jagsi et al, 2012). Based on Medscape’s Physician Compensation Report of 2019 the pay gap is widening in primary care even though patients get better quality of care with women providers (Tsugawa et al, 2017). This is based on studies showing that women are more likely to adhere to guidelines, have more thorough exams, employ more patient-centered care and counseling, and have better patient outcomes when compared to their male counterparts (Tsugawa et al, 2017). The pay gap is intensified for women of color who are paid significantly less than their white colleagues. Implicit bias is more likely at the core of these discrepancies. Also evidence shows that women are more likely to volunteer for unpaid labor, negotiate for lower starting salaries or not negotiate for salary, and have more complex behavioral patients requiring more time and decreasing both volume and quality measures. Still there are areas that, if adjusted, could lead to a significant improvement in the pay gap. These areas include improving salary transparency, increasing mentorship, and restructuring promotion criteria to be more inclusive of the many ways in which an academic physician can excel (Rotenstein and Dudley, 2019). Chat & Chew 0 0 The epidemic of substance use disorder (SUD), particularly opioid use disorder (OUD) including overdose deaths, has impacted all corners of the U.S., and pregnancy is no exception. Most pregnant individuals who develop SUD do so before they even become pregnant (SAMHSA 2018). In 2018, 8 million people over the age of 12 met diagnostic criteria for substance use disorder while about 5.4% of pregnant people reported unhealthy drug use in the last month (Lopari and Park-Lee 2019). The rate of OUD in pregnancy quadrupled from 1999 to 2014 to 6.5 per 1,000 hospital deliveries (Haight et al, 2018) which mimics the rise seen in the general population. However, SUD in pregnancy goes undetected and thus undertreated, compromising both the health of the mother and the developing fetus. Per the Philadelphia Maternal Mortality Review 2010-2012, 50% of all deaths occurred in individuals with a documented history of substance use, while 18% were directly related to drug overdose (PDPH 2015). These findings can be seen throughout the country. To adequately address pregnancy morbidity and mortality in the US, prenatal care providers must be equipped to identify, counsel, and manage substance use disorder in pregnancy in a patient-centered non stigmatizing manner. Per ACOG and USPSTF, universal screening for SUD is an integral component of comprehensive prenatal care. For OUD specifically, pharmacotherapy with methadone or buprenorphine is the standard of care for treatment and leads to improved outcomes for both mother and fetus including decreasing cravings and risky behaviors associated with drug acquisition, decreasing risk of preterm birth and low birth weight, as well as decreasing death by overdose by 20-50% (SAMHSA 2018). For unhealthy use of other substances, pregnant patients should be treated with psychological interventions including therapy and support groups. While the prevalence of SUD increases in pregnancy, physicians need to be well versed in evidence based screening tools, treatment plans for substance use disorder in pregnant patients, and use of non stigmatizing language and counseling.. I am the founder of The Cody Unser First Step Foundation and a member of a passionate committee comprised of primary care physicians (PCP), Spinal Cord Injury (SCI) specialists and persons with SCI, whose mission is to educate and provide resources to PCPs on to the unique needs of persons with SCI. I have personally experienced both physical and attitudinal barriers as someone living with paralysis during primary care appointments. My experiences have been influential in my involvement of education and training of healthcare professionals regarding important health concerns persons with SCI and other disabilities specifically deal with. The Chat and Chews session during the Family Medicine Experience of the AAFP Annual Meeting provides an excellent platform to discuss health care concerns of those with spinal cord injury or disease (SCI/D) from a consumer prospective. Up to 368,000 people in the United States are currently living with SCI/D. Individuals with SCI/D have complex health needs, developing secondary conditions (e.g. cardiometabolic, respiratory, skin, bowel, bladder, sexual dysfunction, pain etc.) throughout their lifespan. They tend to be high users of health care services beyond the post-acute phase of care. Persons with SCI/D often encounter barriers to access to primary, urgent and emergent care, equipment, caregiver supports, and knowledgeable care providers. The medical care of persons with SCI/D is transitioned from specialty providers to the PCP following acute rehabilitation. PCPs are faced with the challenges of treating secondary complications of SCI in health care systems facing increasingly limited resources with regards to patient access to medical care, and provider access to educational and practice support. The current pandemic has further highlighted disparities in SCI care. The Academy of Spinal Cord Injury Professionals (ASCIP) and American Spinal Injury Association (ASIA) have partnered to develop a package of resources to manage the unique health needs of people with SCI/D and the PCPs who treat them. The PCP SCI Healthcare Resource website contains accessible, point-of-care resources that address relevant issues for this population, checklists for both routine periodic evaluations and episodic care, and organizational models to facilitate efficient health care utilization for patients with complex care needs. The website includes a special edition of the journal Topics in SCI Rehabilitation, focusing on issues seen in primary care for people with SCI/D such as: preventative health, sexual health, accessibility/durable medical equipment needs and bowel, bladder and pulmonary complications for people with SCI. In addition to presenting these resources, we will discuss strategies for information dissemination, partnering with SCI consumer organizations, and how to leverage technology to improve care and outcomes. People with diabetes are learning about diabetes products, services and technologies through news, television, social media and peers, often coming to primary care clinics wanting to learn more. How well can you talk about continuous glucose monitoring (CGM) with your patients?

CGM is the standard of care for people with both type 1 and type 2 diabetes on 3 or more doses of insulin daily (by injection, pump or inhaled). CGM provides a visual context to an individual’s current time in range (70-180 mg/dL for most people) and helps the person identify opportunities for health behavior changes and the healthcare professional make more informed therapeutic decisions to increase time in range (TIR).
