Live Experience Details

DAY 1

Expert faculty and peer groups will interact on practice improvements initiatives, Essential Elements of Practice Redesign Case Study, and implementing Practice Transformation (PCMH) standards to understand and improve practice deficits.

DAY 2

Morning: Faculty and fellow physicians will collaborate in groups, organized by the case study deficits established on Day 1. Groups will use Practice Transformation-based solutions to resolve deficits.

Afternoon: Attend an accelerated SAM Working Group on Hypertension, or enjoy the resort amenities.

DAY 3

Morning: Faculty will help each attendee create an online personalized, high-performance action plan to put into practice when back in the office.

Afternoon: Attend an accelerated SAM Working Group on Diabetes, or enjoy the resort amenities.

NOTE:

  • Participants need to bring a computer or tablet to connect to the online modules to begin work on the performance in practice action plan.
  • Pre- and post-live experience course work is required. (practice assessment, patient surveys, and patient chart entry)

**This accelerated SAM Working Group with onsite clinical simulation completion is designed to help you fulfill the knowledge assessment portion and complete the clinical simulation portion of the SAM on the ABFM website during the meeting session. Once completed, you will receive 12 AAFP Prescribed credits per topic area. ABFM Diplomates are responsible for paying all MC-FP fees to the ABFM.
