Freeman spent six years working in nonprofit public health roles, supporting quality improvement and practice transformation efforts alongside physicians serving diverse populations in Chicago, Illinois. She joined the AAFP in April 2017. Freeman is experienced in using a data-driven approach to improve access, continuity, practice efficiency, and efficacy of care delivery. In her current role at the AAFP, she focuses on practice transformation, the Comprehensive Primary Care Plus (CPC+) initiative, and payment models associated with the transition to value-based care.
