CMS recognizes the importance of physicians and other qualified health care providers coordinating care transitions for their patients. Smooth transitions improve care, quality of life, reduce anxiety, help prevent unnecessary readmissions, and reduce total health care spending. For those reasons, CMS pays for transitional care management (TCM) services to facilitate continuity of communication and care to increase the chances patients transition smoothly from hospital- or other health care facility-based care to a community setting.
Two CPT codes--99495 and 99496--are designed to pay for services provided to Medicare beneficiaries whose medical and/or psychosocial problems require moderate- or high-complexity medical decision making during transitions.
