Program Description
Established in 1995, Family Medicine of Southwest Washington is an unopposed, community-based, University of Washington affiliated program sponsored by Southwest Washington Medical Center (SWMC), located in Vancouver, Washington. Bordering the Columbia River, SWMC is a 442 bed healthcare facility serving the Portland Metro and Southwest Washington regions. Besides active Cardiac, Orthopedic, and Neurosciences programs, SWMC has the west coast's busiest ER north of San Francisco. At over 4000 deliveries a year, the Family Birth Center is the second busiest OB unit in this metropolitan area of over 1,000,000 people. FMSW's full time teaching faculty of 11, includes 7 family physicians with OB privileges, 1 Internist board certified in Endocrinology, 1 full time pharmacist, and 1 behavioral scientist. The Family Medicine Center is a 18,500 sq.ft. clinic, and resident "home base". It serves 11,500 patients. While clinic patients represent all walks of life, a significant number are Medicare, underserved, disabled, Latino, and Russian-speaking segments of our community. FMSW faculty and residents provide 35,000 outpatient and 7,000 inpatient visits annually, in a successful, innovative Advanced Access care model, with team-based chronic disease management programs for Diabetes, Asthma, and Medical Management of Chronic pain. We also do group visits. With the anticipated “go live” of GE Centricity EMR in the late Fall of 2009, FMSW will have completed integrating all the elements of the TransforMed Medical Home model. Our residents have been trained in the “New Model” since 2004. FMSW is a rich learning environment. Residents enjoy autonomy with support, consultation with enthusiasm, mentoring with care from both community physicians and core faculty, and the opportunity to teach medical students on core, elective and Sub-I rotations. FMSW continues to champion a spirit of community involvement through various major annual initiatives, including: • Sports Medicine Program providing more than 600 low-cost physicals for high school athletes (Summer) • Breast/Cervical and Prostate screening programs for the community (Fall) • Team Physician services for local high school football games (Fall) • Free Community Clinic for the underserved and homeless (weekly) • Free office procedures for the uninsured, supported by a Foundation grant FMSW takes pride in training great family doctors for today's complex practice environment. While some program graduates practice in Alaska, Arkansas, California, Idaho, Montana, Mississippi, Oregon, and Texas, most practice in various rural, urban and suburban Washington communities. Almost half of our residents have remained in the Portland-Vancouver Metro area.
