Apply For Membership
If you are a practicing physician, the following information may be necessary to complete your application.
- Medical license information (license number, issuance date, and expiration date)
- Credit card information (Membership dues and/or selected contribution(s) will not be charged until your application is approved.)
Note: If Active membership was held within the last 24 months, you must have documentation of 100 CME credits earned during the same time period on file with the AAFP. To confirm that you have the necessary credits on file or to report additional credits, please contact the AAFP by email or at (800) 274-2237. Our staff will be happy to assist you.
Note: If you are a residency program director or residency coordinator and wish to enroll your residents as AAFP members, please log in to the Residency Portal and click on the “Request Memberships” button under Residency Tools. If you need assistance, please contact the AAFP by email or at (800) 274-2237.
If you do not have an AAFP account, you must create one before applying for membership.
