The American Academy of Family Physicians (AAFP) is recruiting family physicians to present their best practices and lessons learned at the Adolescent Immunization Best Practices Summit. The emphasis of the summit is to have physicians summarize how their practice increased adolescent immunization rates through the operation of a quality improvement program or project. This will also include implementation of a team-based approach and sustainability plan within the practice.

The selected family physicians will participate in the Adolescent Immunization Best Practices Summit on July 14-15, 2018, in Kansas City, Missouri.  The selected family physicians will present their best practices and lessons learned at the summit.

The emphasis of the summit is to have physicians summarize how their practice increased adolescent immunization rates through the operation of a quality improvement program or project. This will also include implementation of a team-based approach and sustainability plan within the practice. All the selected physicians will have the option to present their findings in the following two formats:  10 to 15-minute panel session with PowerPoint slides and/or a poster presentation. Both presentation options will include time allocation for questions/answers.
