The Medicare Access and CHIP Reauthorization Act (MACRA) permanently repeated the flawed sustainable growth rate (SGR) and set up the two-track Quality Payment Program (QPP) that emphasizes value-based payment models. Physicians will choose between two payment tracks: the Merit-based Incentive Payment System (MIPS) track or the Advanced Alternative Payment Model (AAPM) track.The MIPS track replaced three existing quality programs and added a new performance category. MIPS and AAPMs are collectively referred to as the Quality Payment Program (QPP). The first performance period for the QPP was 2017.

Most family physicians will initially participate in MIPS. Many aspects of Medicare's previous quality programs you participated in were rolled into the four performance categories under MIPS.

2019 Pathway to QPP

According to the rule, performance in 2019 will determine payment adjustments in 2021.
