Spontaneous sustained VT

Cardiac arrest related to VF or VT not resulting from a transient or reversible cause

Syncope of undetermined origin with clinically relevant, hemodynamically significant sustained VT or VF induced at electrophysiologic study when drug therapy is ineffective, not tolerated or not preferred

Nonsustained VT with CAD, prior myocardial infarction, left ventricular dysfunction and inducible VF or sustained VT at electrophysiologic study that is not suppressed by a class I antiarrhythmic drug
