A	Patients at high risk of developing heart failure
B	Patients with cardiac structural abnormalities or remodeling (e.g., left ventricular dysfunction from a prior infarction) who have not developed heart failure symptoms
C	Patients with cardiac structural abnormalities or remodeling and current or prior symptoms of heart failure
D	Patients with refractory end‐stage heart failure
