Review your MIPS score and payment adjustment by the deadline

October 21, 2026

Once again, it’s time to review your Merit-based Incentive Payment System (MIPS) performance feedback and final scores. The Centers for Medicare & Medicaid Services (CMS) recently released the 2025 reports, which CMS will use to determine whether MIPS participants get positive, neutral, or negative Medicare payment adjustments next year. CMS expects to release MIPS payment adjustment amounts later this month.

How to access your report

You can access your score and performance feedback by signing into the Quality Payment Program (QPP) website with the same credentials you used to submit your 2025 MIPS data. Practice representatives can view individual, subgroup, and group performance feedback and payment adjustments. Representatives of alternative payment model (APM) entities and virtual groups can view information at the entity and group level, respectively. Third-party representatives cannot access feedback and scores.

Participants in Medicare Shared Savings Program accountable care organizations (ACOs) should contact their ACO to find out how to obtain a HARP account via the ACO Management System. ACO representatives must have the “staff user” role to be able to access scores for the entity.

What to do if you find errors

If you believe CMS erred in calculating your final score or payment adjustment, you can request a targeted review through the QPP website. The targeted review period opened on Sept. 21, 2026, when CMS released the final scores, and will remain open for approximately 60 days. Circumstances that may warrant a targeted review include the following:

  • You submitted data under the wrong tax identification number or national provider identifier.
  • You have qualifying APM participant status and should not be subject to MIPS payment adjustments.
  • Your performance categories were not automatically reweighted even though you qualified for reweighting due to extreme and uncontrollable circumstances.

MVP deadlines

CMS intends to sunset traditional MIPS after the 2028 performance year. At that point, the primary reporting option will be MIPS Value Pathways (MVPs); the APM Performance Pathway (APP) is for MIPS APM participants only.

The MVP reporting option requires registration during the designated window, which closes Nov. 30, 2026, at 8 PM ET. Register on the QPP website, or find additional information in the 2026 MVP Registration Guide.

CMS is also seeking feedback on the MVP reporting experience. The MVP Adoption Survey is now open and takes approximately 15 minutes to complete.
