A checklist for better dementia care plans

Limited diagnosis and treatment options may leave you feeling like there is little that can be done to help your patients with dementia, but clinical tools such as a dementia care plan checklist can help you to focus on the cognitive, functional, and behavioral losses that most affect their quality of life.

A dementia care plan organized around these four core components can help to achieve patient-centered goals:

• Defining and acting on cognitive problems,

• Managing the neurological, mental health, behavioral, and functional problems that result from the disease,

• Addressing relevant co-morbid conditions,

• Providing caregiver assistance.
