• A checklist of quality measures to help you succeed in value-based payment

    Because different payers require different quality measures, keeping track of them can be a serious challenge for physicians and their staff. This quality measure checklist can help.

    The checklist integrates measures from three widely used sources: the Core Quality Measures Collaborative, the Integrated HealthCare Association’s Value Based P4P program, and the Healthcare Effectiveness Data and Information Set (HEDIS).

    The measures are organized by age and disease type under four major categories:

    • Screening, prevention, and medication safety,

    • Acute care,

    • Chronic care,

    • Older adult care.

    Before seeing a patient, simply scan the checklist for any relevant measures, and pick at least one to address at the current visit.
